Please know that you must inquire separately with your physician and associated providers to find out if they are out of network. Your surgeon and other healthcare providers, including anesthesiologists and pathologists, may provide services to you at the center that are not part of the center’s fees. If any of their services are provided to you while being a patient of the center, the provider (s) will bill separately for their services rendered.
Patients should contact each healthcare practitioner who will provide services in the center to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider. The patient and their provider(s) are responsible to elect and determine which services and planned procedures will be authorized to perform based on their intended payment plan and policies. You should contact your health benefits plan for information regarding your copayment, deductible and/or coinsurance amount. Contact information is typically found on the card provided to you by your health plan. The center’s insurance verification department is always available as a resource for pre-procedure assistance.
